State of WI- SSA Disability Determination Bureau

Cassandra Quinn

The Wisconsin Disability Determination Bureau (DDB) provides disability decisions for Social Security Disability and Wisconsin Medicaid disability applicants. As part of the disability process, the DDB requests medical and other records from facilities throughout the state and beyond, and strives to form a collaborative relationship with HIM Professionals for efficiency in that process. Did you know you can submit records electronically via Electronic Records Express (ERE)? Visit for further information or contact the Professional Relations Officers at 608-266-1565 with any questions or to establish your ERE account.
